Here are the current SCAT3 tools in pdf format (Sports Concussion Assessment Tool)

The adult SCAT3 tool is for ages 13 and up, the child SCAT3 is for under 13 years old

Well worth reading through as presentation to the ED following concussion is very common.

Also remember that about half of all concussions are not sports related!
